New patients using workers' compensation insurance must bring the following

It is the patient's responsibility to confirm that we participate with his/her insurance plan and also to bring a Primary care physician (PCP) referral, if required by your insurance.
Please arrive 20 to 25 minutes before your initial appointment to allow time for paperwork.
You may arrive on time for your initial appointment if you bring the completely filled and printed forms.

CANCELLATION POLICY
$80.00 fee will be charged in the event you are unable to attend your appointment, unless you provide a 24 hours advanced notice (between Monday – Friday between 8am to 5 pm) prior to your appointment. $80.00 fee will be also be charged for late arrivals (10 minutes or more), missed appointments and appointments not cancelled or rescheduled 24 hours (between Monday – Friday between 8am to 5 pm) prior to the scheduled appointment time.
INCLEMENT WEATHER POLICY
