NEW PATIENTS

  • New patients using insurance must bring the following
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Insurance Card(s)
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
    • Primary care physician (PCP) referral, if required by your insurance
  • New patients using workers' compensation insurance must bring the following
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Claim Number
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
  • It is the patient's responsibility to confirm that we participate with his/her insurance plan and also to bring a Primary care physician (PCP) referral, if required by your insurance.
  • Please arrive 20 to 25 minutes before your initial appointment to allow time for paperwork.
  • You may arrive on time for your initial appointment if you bring the completely filled and printed forms.

CANCELLATION POLICY

  • Missed Appointments/Cancellations/Rescheduling requests that occur less than 24 hours before the scheduled appointment are subject to a $80 fee in all circumstances, including illness.  Late arrival will shorten your treatment time. Cancellation/rescheduling requests must be submitted via e-mail or voicemail if you are unable to reach us by phone.  Arrival fifteen minutes or more after your treatment time will be considered as a “no show” and the cancellation fee will apply.
