WE DO NOT PARTICIPATE WITH Aetna, Anthem Health Keepers, Medicare Advantage Plans, Medicaid, Humana, Kaiser & NON PPO PLANS OF ANTHEM & CAREFIRST.
We participate with PPO Plans only (does not apply to Medicare, FEP, GEHA, Tricare & Workman's Comp Plans). PPO plans do not require authorization or reauthorization for therapy. We do not file authorization or reauthorization requests.
It is the patient's responsibility to verify with his/her insurance (that a prior or continued authorization is not required to attend therapy). All patient's with insurances that require prior or continued authorizations will be billed SELF-PAY rates.
